Hispanic Breast Cancer Survivors' Perception of Health Affected by ...

  • Limited proficiency in English was linked to lower personal perception of health.
  • Effect was absent in those able to manage their illness.
  • Higher ?spiritual well-being? weakened the impact of language proficiency.

SAN DIEGO ? Hispanic breast cancer survivors who considered their English-language proficiency as limited were less likely to rate their own health as good, very good or excellent, according to data presented at the Fifth AACR Conference on The Science of Cancer Health Disparities, held here Oct. 27-30, 2012. However, the association between English-language proficiency and self-rated health was not found in women who had higher spiritual well-being, as measured by their sense of peace or meaning.

Self-rated health is a single-item question that asks people to rate their health on a scale of 1 to 5, with 1 being poor and 5 being excellent, according to the researchers.

?Even though it is a single-item question, in large epidemiological studies of mostly non-Hispanic white participants, it has been shown to be highly predictive of morbidity and mortality in chronic disease,? said Maria Garcia-Jimenez, a student at the University of California Berkeley/University of California San Francisco Joint Medical Program.

To examine the association between various acculturation measures, particularly English-language proficiency, and self-rated health, Garcia-Jimenez and colleagues conducted a secondary analysis of data from a telephone survey of 330 breast cancer survivors who identified themselves as Hispanic.

The average age of the women surveyed was 58.3 years. Nearly all of the women had been diagnosed with breast cancer within the past five years. About 40 percent of the women reported speaking English not at all, poorly or fairly well.

About 25 percent of the women self-rated their health as poor or fair. Those women who were proficient in English were more than twice as likely to rank their health as good, very good or excellent compared with women with limited English-language proficiency.
The researchers then evaluated whether cancer ?self-efficacy,? or an individual?s perception of their ability to manage the repercussions of their disease, attenuated these findings.

?We found that if women had greater cancer self-efficacy, the effect of language on self-rated health was eliminated,? Garcia-Jimenez said. ?Regardless of their language ability, having a high cancer self-efficacy was associated with higher self-rated health.?

In addition, researchers examined two elements of spiritual well-being, faith and sense of peace/meaning, and their effects on self-rated health. Data indicated that faith was not associated with English-language proficiency or self-rated health, but the sense of peace/meaning was highly associated with self-rated health, according to Garcia-Jimenez.

Genetics researchers at the University of Adelaide have solved a 40-year mystery for a family beset by a rare intellectual disability and they've discovered something new about the causes of intellectual disability in the process.

While many intellectual disabilities are caused directly by a genetic mutation in the so-called "protein coding" part of our genes, the researchers found that in their case the answer laid outside the gene and in the regulation of proteins.

Protein regulation involves the switching on or off of a protein by specific genes. As a consequence in this case, either too much or too little of this protein can trigger the disability.

The team has studied a large (anonymous) Australian family of 100 people, who for generations have not known the source of their genetically inherited condition.

The disability which results in a lower IQ, behavioral problems such as aggression, and memory loss, and is linked with developmental delays, epilepsy, schizophrenia and other problems affects only the male family members and can be passed on by the female family members to their children.

Genetic samples taken from the family and laboratory testing involving mice have confirmed that the protein produced by the HCFC1 (host cell factor C1) gene is the cause of this disability.

"The causes of intellectual disability generally are highly variable and the genetic causes in particular are numerous. The vast majority of intellectual disabilities are due to genetic mutations in proteins, so it was rather unexpected that we found this particular disability to be due to a regulatory mutation," says the leader of the study, Professor Jozef Gecz from the University of Adelaide's School of Pediatrics and Reproductive Health.

"We've been researching this specific disability for 10 years and it's taken us the last three years to convince ourselves that the protein regulation is the key," he says.

"For the family, this means we now have a genetic test that will determine whether or not a female member of the family is a carrier, which brings various benefits for the family.

"From a scientific point of view, this widens our viewpoint on the causes of these disabilities and tells us where we should also look for answers for those families and individuals without answers.

"This is just the tip of the iceberg in understanding the impact of altered gene regulation on intellectual disability the gene regulatory landscape is much bigger than the protein coding landscape. We have already found, and I would expect to continue finding, a number of other intellectual disabilities linked with protein regulation over the next 20 years or so."

Can Controversial Ocean Iron Fertilization Save Salmon?

What's been described as a "rogue" geoengineering experiment is really an effort, however flawed, to restore salmon abundance

In a bid to restore lost fish abundance, the Haida Salmon Restoration Corp. (HSRC) undertook to mimic the effects of a volcanic eruption by fertilizing the ocean with iron. The idea was to provide the missing nutrient for a plankton bloom that would then trickle up the food web and restore salmon?with the ancillary effects of gathering data on the ocean food web and, potentially, removing carbon dioxide from the atmosphere.

"What if this is a means by which ocean pastures can be stewarded and brought back to health?" asks Russ George, chief scientist of the expedition as well as a controversial businessman with a history of attempting to start CO2-removal schemes ranging from reforestation to ocean fertilization. "This is a tiny village of people trying to take care of their backyard."

Old Masset village on the Haida Gwaii Islands off the British Columbia coast did this by contracting George and others to initiate the largest such intentional ocean fertilization effort to date. It authorized the release of roughly 110 metric tons of iron dust, 91 metric tons of the iron sulfate fertilizer commonly used as a lawn treatment and employed in other scientific experiments, and nearly 20 metric tons of the iron oxide found in soils around the world. "It's micronutrient enrichment," Jason McNamee, operations officer and corporate director for the HSRC, told a press conference on October 19. "We took a bag of iron, and we slapped it over one square kilometer [of ocean]."

To do that, this past July the HSRC team motored more than 300 kilometers west from the Haida Gwaii Islands to an ocean eddy in the fishing vessel Ocean Pearl. The area had previously been scouted by collecting water samples since January and lies outside Canadian territorial waters. The team also used more than 20 autonomous oceangoing robots, including two bright yellow gliders and 20 Argo drifter robots on loan from the U.S. National Oceanic and Atmospheric Administration, to survey the scene?work that is ongoing. Over the course of several days the researchers then released the 110 metric tons into the fishing boat's wake in an attempt to raise the levels of iron in the water from one or two parts per trillion up to five to 10, although both concentrations are estimates.

Satellite images as well as maps of chlorophyll abundance appear to show that the iron did indeed fuel a plankton bloom in August. But questions remain: Was it the right kind of bloom to bury carbon? Will it have any effect on the salmon?

Ocean restoration
The HSRC's basic idea is born out of marine biology as well as observations of the aftermath of volcanic eruptions in the region. For example, a lack of iron limits the growth of microscopic plants in the Southern Ocean around Antarctica and elsewhere, a fact that prompted marine biologist John Martin to famously muse: "Give me half a tanker of iron, and I'll give you the next ice age."

In the summer of 2008 Mount Kasatochi in Alaska's Aleutian Islands blew, sending volcanic minerals, including iron dust, far to sea and prompting plankton blooms across the Gulf of Alaska. At the same time, years and years of decline in returning salmon populations led researchers to expect few of the fry from 2008 to return in later years, only to find a record salmon run in Canada's Fraser River in 2010.

Lawyer for China's deposed Bo unsure if he can take case

BEIJING (Reuters) - A lawyer for disgraced former top Chinese politician Bo Xilai, who has been employed by the family to represent him, said on Thursday he was unable to say whether the government would allow him to represent Bo when the case comes to trial.

Bo's wife, Gu Kailai, and his former police chief, Wang Lijun, have both been jailed over a scandal stemming from the murder of British businessman Neil Heywood while Bo was Communist Party chief of the southwestern city of Chongqing.

The government last month accused Bo of corruption and of bending the law to hush up Heywood's murder. Before Bo is formally charged and tried, investigators must complete an inquiry and indict him.

Bo's mother-in-law, Fan Chengxiu, has retained Li Xiaolin, who was also part of Gu's legal team, to represent him.

But Li said he was unsure if the authorities would allow this. During Gu's trial, she had to use government-appointed lawyers.

"I still can't say whether I'll be able to get involved in this case. We will try our hardest to take it," Li told Reuters in a brief telephone interview.

Li said he did not know where Bo was and added that he had not seen him, as he had only just been employed by Fan.

"There are many things that are impossible to know. I know about as much as you do," Li said.

He declined to speculate on when a trial may happen.

"How can I estimate this? It's all guesswork," Li said.

China is expected to expel Bo from parliament on Friday, which would strip him of the immunity from prosecution he had enjoyed as one of its members, paving the way for formal criminal charges to be laid.

As China's prosecutors and courts come under Communist Party control they are most unlikely to challenge the accusations against him.

The Bo scandal has overshadowed preparations for an important party congress that opens November 8 and will unveil the country's new central leadership line-up.

Bo, 63, was widely seen as pursuing a powerful spot in that line-up before his career unraveled after his former police chief fled to a U.S. consulate in February, saying that Bo's wife had poisoned Heywood.

Late last week, Moog outed its LEV-96 sensoriactuator prototype and offered a glimpse at its latest R&D unit. Even though it's still in the early phases of beta-testing, we were able to stop by the Moog Music factory for a closer look and a brief glimpse of the gear in action ahead of its appearance at Moogfest. While the unit is installed on acoustic guitars for the time being, the company says that similar tech can be used on other acoustic instruments and eventually to other surfaces -- this is just the current manifestation. Since the tech modifies the guitar's natural harmonics and string vibrations, the LEV-96 is getting cozy on both traditional acoustic guitars and those outfitted with pickups in its present state.

As far as controls go, the entire unit is capacitive touch-enabled from the moment a finger swipe powers it on. Sliders allow for adjusting the intensity, harmonics and note duration while the other buttons enable arpeggio presets and modulation that includes tremolo and random harmonic tweaks. Those sliders remain in play when a preset is activated, serving to enable further adjustments on selected There is a lock button, too, so that you don't accidentally make a switch mid-strum. All of these finger-friendly surfaces work alongside two pairs of electromagnetic pickup channels per string to wrangle the 96 simultaneous harmonics. Magnets work to either increase of decrease the string's motion, bringing out vibrational modes that have always been in-play on acoustic instruments, but have never been offered the power needed to make 'em sing. The folks at Moog are quick to remind us that the LEV-96 is still in its infancy, but you can rest assured we'll be keeping an eye our for what develops. PlayMakers Repertory Company will present the world premiere of David Ball?s adaptation of Moli?re?s 17th century comedy The Imaginary Invalid, on Oct. 24-28 and Oct. 30-Nov. 4 and Nov. 6-11 in the Paul Green Theatre in the University of North Carolina at Chapel Hill?s Center for Dramatic Art. This timely excoriation of unscrupulous elements of the health-care industry was commissioned by UNC?s professional-theater-in-residence.

?Imaginary Invalid is almost always funny,? notes dramatist David Ball. ?I wanted my adaptation funnier and closer to the intersection with tragedy than others I?ve seen.?

Dominique Serrand, who is co-founder and artistic director of Theatre de la Jeune Lune of Minneapolis, MN, confesses, ?[Imaginary Invalid] is a haunting play for any French director to tackle, being so loaded with the history of its original production.? He adds, ?[It is] Moli?re?s last [play], at the performance of which he died [on Feb 7, 1673], ironically playing the Invalid himself. A bit like walking under a ladder, it is the Moli?re play at the end of which we, as directors, wonder if this could indeed be our last one as well.?

Serrand says, ?The haunting character of the play is certainly one of the defining elements that did provoke me to do it. It is also the immense pleasure to work on a writer?s very last play. Not necessarily thinking about categorizing it within the entire repertoire (is it his very best play? probably not), but admiring the evolution and the richness of invention accrued over all the years of writing.?

He claims, ?I am not looking at The Imaginary Invalid as a single play, but as an extraordinary achievement of the writer of Tartuffe [1664], Don Juan [1665], and other great human comedies. The man who stands there in front of you, the Imaginary Invalid named Argan, tortured by the thought that he might be sick although he is not, is performed by Moli?re, very sick, who actually dies at the end of his own performance of the character he wrote. [The Imaginary Invalid is] A comedy deeply grounded in tragedy.?

PlayMakers guest director Dominique Serrand will stage The Imaginary Invalid with his long-time collaborator Steven Epp in the title role of the rich hypochondriac who employs a veritable flock of quack doctors to treat his imagined illnesses.

Playwright David Ball says, ?The chance to work with either director Dominique Serrand or actor Steven Epp is a privilege; the chance to work with both at once is a privilege I could never pass up. Dominique is one of the few true geniuses directing in American theater. Steven is so outrageously good that I would write any role for him he ever wanted to do. Just having a script of mine done is no longer important to this old guy ? been there done that a hundred times ? but tempt me with Serrand and Epps and I?m all in.?

In The Imaginary Invalid, David Ball says, ?A hypochondriac named Argan (Steve Epp) lets his monomania take over his life; he depends on quack doctors and a smorgasbord of enemas to keep him alive. He wants his daughter [Katie Paxton as Little Angel] to marry a famous doctor, so there?ll be a doctor right in the house. But his daughter is in love with someone else. It?s a fun and commonplace little plot that opens the door to a larger world of obsession, sanity, and a sobering confrontation with Death.?

Dominique Serrand says the show?s set is ?a scenographic set of spaces creating dynamic and great flow for the play?s surprising happenings?. The lighting is a long gesture, instead of a series of effects[, and the costumes are] Contemporary [but] influenced by the period.?

He also cautions Triangle theatergoers that ?The play will include strong language.?

In adapting Moli?re?s final play for a 21st century audience, David Ball says, he faced three major challenges: ?(1) Replicating for modern audiences the outrageous shock value the original had on its audiences. (2) Getting past the expectation that Moli?re just wrote funny plays without tragic foundation, and accomplishing this without losing the play?s endless comedy. (3) Since French and its robust overtones cannot be translated into American English, finding ways for the robustness of American English to make up for what translation from French necessarily removes.?

Dominique Serrand says that biggest challenge as a director staging The Imaginary Invalid is: ?Respecting the playfulness and the original vision of Moli?re in the performance, including Moli?re?s own death. He was very sick as he appeared onstage playing the lead character.?

Iraq?s oil could reshape global energy landscape

?

NEW YORK --- In a major new study published last week, the International Energy Agency reconfirmed what Washington has long suspected: Iraq has the potential to reshape the global energy landscape in the years ahead, thanks to its huge untapped oil reserves.? But whether Baghdad can capitalize on this opportunity is far from clear. The stakes are high?both for the global economy and the country?s future.

Despite decades of turmoil and bloodshed, Iraq is already one of the world?s major oil suppliers. The roughly 3 million barrels a day it pumps make it the world?s third-largest exporter. Consider that Iran, hobbled by Western sanctions, is only producing half as much oil today as Iraq, whose wells are putting out more than twice what they did in 2003, the year of the Iraq War.

Yet by the 2030s, according to the IEA, Iraq may double its current output, leapfrogging energy-powerhouse Russia as the second-largest oil exporter in the world. This is hardly a far-fetched forecast. The country?s proven oil reserves are the fifth largest in the world, its proven gas reserves the thirteenth largest. Its actual rank is likely far higher.

In comparison to other major oil producing countries, Iraq is still uncharted territory. Much of its geology remains little known and may well hold significant additional amounts of oil. A good part of what has been explored, at least outside of the Kurdistan area, happened prior to 1962. Today?s vastly better technology and higher oil prices almost certainly mean that sizeable new reserves will soon be discovered.

Getting this oil in the ground to market will not be easy, though. Iraqi leadership will need to remove several roadblocks. The first is infrastructure. As it stands, essential hardware like pipelines and export terminals are inadequate to transport much more oil to tankers waiting offshore. Political improvements will also be required, such as building up the country?s regulatory framework and legal processes. Some problems are simply logistical. Coordinating the efforts of a vast team of publicly traded Western oil companies, state-owned firms, Iraqi bureaucrats, and countless service providers will be essential. And on top of it all, security challenges loom large. Though the incidence of violence in the country has fallen markedly over the last five years, energy personnel and vital infrastructural will need protection for Iraq to ramp up its oil and gas industry.

The economic windfall the country could reap from an energy boom is substantial. The IEA estimates that oil exports could add a cumulative $5 trillion to its economy over the next 20 years, a godsend the agency calls a ?transformative opportunity.? The results could be impressive. The country?s gross domestic product (GDP) would be five times what it is now if this growth materializes. Per capita, the Iraqi economy of 2035 would be roughly on par with where Brazil is now.

Yet there are risks. Oil revenues can cause a country more problems than they solve. If not properly reinvested, black gold can wreak havoc on other parts of a nation?s economy, feed corruption and militarism, and leave a country painfully vulnerable to sudden fluctuations in commodity prices. A thriving oil and gas industry, by itself, will not be nearly enough to provide jobs to the country?s growing population; other sectors must also become competitive. Moreover, unless the Iraqi government can tackle the host of above ground issues plaguing its energy industry, the riches that lie below ground may be very slow to arrive.

If Iraq can ramp up its oil production, American consumers will be among the winners. A dramatic rise in US oil production over the last five years has made a significant contribution to global supplies, but Iraq will likely be the single biggest source of new oil over the next two decades. The Middle Eastern country?s oil will be crucial for feeding growing global demand. Iraqi export levels are likely to play a decisive role in whether oil supplies will be enough to keep a lid on prices. If Baghdad disappoints, Americans will feel it at the gas pump.

Iraq is in a unique position to help take the edge off a global oil market under serious strain. Time will tell whether the country will achieve its lofty goals?or if they will remain a mirage.

Conversation with President Faust

The speaker for this month?s ?Conversations with Kirkland? series was none other than Drew Faust, president of Harvard University. On October 17, in an intimate setting with no more than about 30 students, President Faust answered a set of questions prepared by two moderators and an additional few from members of the audience.

The event was held in a town-hall style, with President Faust jumping from issue to issue dictated by the curiosity of the students. The questions touched upon more than a dozen issues, from her latest book on the Civil War to Harvard?s Allston development project. Nevertheless, President Faust?s answers to these diverse questions also revealed a couple of common themes that perhaps represent her vision for the university:

Learning for the Sake of Learning

As expected, one of the beginning topics of the Q&A was academic integrity. Faust navigated her thoughts on this issue well, pointing the audience to the root of the problem at hand. Speaking to the unhealthy amount of pressure that grades and GPAs put on Harvard students, Faust emphasized feeling ?pleasure and satisfaction in work and learning? and said that it is crucial to ?recognize how important the process of doing our own work is.?

A common theme ran in her answers about the common space construction projects all throughout campus. Currently, the area around the Science Center is fenced off for construction and the administration is looking to expand student space to the ground floor of the Holyoke Center. She described the beauty of having common spaces as ?letting everyone take advantage of everyone else,? in terms of sharing ideas, intellectual passions, and experiences.

Faust?s answers to both questions successfully opened the window to her vision of Harvard as a place where knowledge by itself is appreciated, respected, and shared, regardless of the grade or reward these intellectual pursuits would result in. No matter the sour smell or the dusty air around the Science Center coming from the construction work, the intentions behind such projects are owed applause and support.

Harvard as a Leader in the International Community

Faust said that what surprised her most when she became president of the university was ?how much in the public eye Harvard is.? She added that while the amount of attention could be ?disquieting,? it was also an ?opportunity? for Harvard to become a global leader in higher education.

Faust?s most famous achievement from the past five years of her presidency is most likely her initiative to significantly increase financial aid at the College. She said that such support for the financial aid program is needed to ?support people from every possible background,? and to ?sustain the level of intellectual excellence? at the university.

Meanwhile, Faust also said that the financial aid initiative also raised questions about the ?balance of funds between financial aid and other initiatives at the university.? She said that the administration is constantly looking for ways to make ?instruction within Harvard less costly.? This would involve the use of technology and other advanced teaching tools, opening up another opportunity for Harvard to lead the world in higher education improvement.

Where She Fell Weak

While Faust gave clear answers and had a unified vision for the university, there were also places during the Q&A where her answers were not as strong. These answers were also in response to questions about three of the most heated issues within the university, namely female involvement in academia, student mental health, and public service.

In response to a question about women and professorships, Faust gave a conventional and generic answer that emphasized ?support for childcare? and encouraged ?more women and minorities into the pipeline.? However, pipeline argument has been proven, time and time again, to fail in increasing the percentage of females in university faculties. Faust didn?t touch on the root of the problem preventing women from entering and being promoted in academia no matter how many of them have come through the ?pipeline,? such as maternity and sexual harassment, just to name a few.

The same problem persisted in her answer to a question about increasing interest in public service on campus and after graduation, in which she said that students need to be shown ?a smoother path to public service? in the face of more financially assuring careers provided by the financial and consulting sectors. However, Faust did not specify how this would be done or any specific potential initiatives that would make this possible.

Lastly, Faust did not comment deeply on mental health, an issue that came up two times during the event. She only reinforced the core problems within the larger issue, saying that ?there should be no stigma about reaching out for health,? and pointed to efforts being initiated by Dean Hammonds, such as the new Committee on Student Stress.

The Q&A ended on a lighter note that a moderator asked Faust a question about her ?dream Secret Santa,? to which she replied ?no more outdoor Harvard celebrations in the rain? ? a reference to the dampened 375th celebration last fall. While Faust failed to give substantial answers to questions about many of the central issues in the Harvard community, she also provided a relatively candid and remarkably clear vision for the school as shown by the common themes running throughout most of her answers.
